The following forms can be downloaded, printed, signed and returned to the school office.

Forms you need to send BEFORE the first day of school:

State Required Health Appraisal – please make doctor appointments now, it can take up to 6 weeks to get a
well-child appointment with your child’s physician.

State Required Vision Screening

Certified copy of Birth Certificate – if you do not have a certified copy of the birth certificate please order a
copy from the court house in the county your child was born.  This process can take as long as two months
to receive your certificate and is required by the State of Indiana.

Baptismal Certificate (Catholic students that were not baptized at St. Mary’s Church)

Forms not required but strongly encouraged
Physical Exam
Dental Check-up
(Although these forms are not required by the State of Indiana, we feel it is in the best interest of every child to
have these completed prior to the child beginning kindergarten.  We encourage you to have these exams
completed prior to the first day of school.)
